Patient Pre-Registration Information

Our friendly and courteous registration staff is here to serve your registration needs whether you are here for a simple lab test, x-ray test, same day surgery, or inpatient services.

Registrars are most often your first contact when you come to the hospital for treatment or services. They are responsible for setting up your visit information so that caregivers have basic information about you to properly care for you. Admissions staff may request identification and will obtain general written consent for treatment from you. In addition, registrars collect financial information from you so that we may accurately bill you or your insurance for services rendered. You are also advised of your rights and responsibilities as a patient under State law, offered a copy of our Notice of Privacy Practices as required by federal law, and informed of your rights and obligations as a Medicare or Tricare beneficiary (where applicable).

The Patient Access Department is located in the Main Lobby of the hospital. Hours of operation are Monday - Friday - 6:00 am – 4:30 pm

For your convenience, there is also a Patient Access Representative located in the Laboratory, Monday-Friday – 8:00 am – 4:30 pm

Emergency Department Registration Representatives are available 24 hours per day, 7 days per week.

Here's a list of things to bring that will make your stay a little easier.....

  • List of medications, including the last time you took a dose of each. (don't forget to include over-the-counter medications, non-prescription, as well as herbal, vitamin and dietary supplements.)
  • All medications in their original containers
  • Copies of advance care documents
  • A designated driver if you'll be going home following a procedure in which you will receive sedative medications.
  • Cosmetics and toiletries if you prefer to use specific brands rather than those provided by the hospital
  • Comfortable clothing for your return home
  • A list of all allergies you have, as well as any other reactions you have had to medications
  • A current immunization list, including pneumonia, influenza and tetanus vaccinations
  • Information regarding implants (artificial joints, pacemaker, rods, screws, plates) or donated organ/tissue surgically placed in your body.
  • Names and numbers of people to contact in case of emergency
